	Registration Information Registration for the 2008-2009 school year is available from 9:00 a.m. until noon. If you are coming from a school within Broward County you will need: *Two proofs of residency: 1. Closing papers from your home purchase or a copy of your lease. 2. Copy of utility bill, such as electric, water or cable. If you are coming from outside of Broward County you will also need the following: • A copy of your child's birth certificate • A copy of his or her Standarized Test Scores • Most recent report card • A current physical, signed by the doctor with this I.D. number, dated within the last 12 months • Florida 680 Health Form which lists your child's immunizations
